Application form :
Application form needs to be hand written with blue pen by the traveller. All the jurisdiction forms are uploaded as a single pdf.

Hotel bookings :
Provide confirmed hotel reservations covering your travel dates.
Include reservations for other Schengen countries if you’re visiting them.

Previous visas :
Include front and last page copy
Provide copies of previous Schengen Visas.
Include copies of valid US and UK Visas if applicable.

Flight bookings :
Copy of flight bookings for the whole duration of the trip and
Proof of any other internal travel (bus/train/flight) reservations.

Passport size photo :
Two recent passport-size photos

IT return :
Include ITR acknowledgement for the last three assessment years or Form 16A
Should show a barcode.

Travel insurance :
Get travel insurance with coverage of at least 30,000 Euros.
Mandatory for a minimum of 10 days from your travel date.

Address Proof : If Passport issued location is different from your current location: Provide address proof like a letter from your bank/employer, rent agreement and electricity bill or property papers if your passport was issued in a different jurisdiction.

Appointment letter :
Download and upload the appointment slip on your Teleport application.
Carry it on your appointment day.

Passport front page :
Ensure it’s valid for at least 3 months after your intended stay.
Must have at least 2 blank pages.
Provide two sets of photocopies of the passport.
Issued within the previous 10 years

Bank statement :
Submit your own recent bank statement, covering the past three(6) months, with an official stamp and signature from the bank.
For Delhi jurisdiction, include 6 months’ statements.
The statement should be no more than one week old at the time of application.
Recommended bank balance is at least 150 euros per person per day.

Company IT return : Include the latest 3 years’ company Income Tax Returns.

Cover letter : Mention your name, travel details, expenses, and purpose of the visit.

Salary Slip : Provide your latest 3 months’ pay slips.

Letter From Indian Company : Letter from Indian Company or Institution, explaining purpose of the visit, professional status of applicant, and details regarding activities and business relation with Italian company. The letter must be signed by the authorized person, whose name and charge must be clearly identified. 

Employer NOC : Include a No Objection Certificate (NOC) with leave dates from your employer on company letterhead.

Marriage certificate : Marriage certificate is required

Company Registration : Include full business registration documents, such as Certificate of Incorporation or GST.

NOC Letter From Parents :
For minors travelling with one parent or a third party. the parent(s) must provide a notarized and legalised affidavit stating they have no objection to the minor’s travel. If the parent resides in Italy, they can make a similar self-declaration with a copy of their stay permit and passport, authenticated by the Commune.
An alternative option is for parents to sign a consent form at the Consulate General of Italy, Mumbai. This requires their presence along with a copy of their passports (any government photo ID with signature) and the minor’s birth certificate.
